Silver Care LLC provides Supportive In-Home Services for adults and children. Residential services in the individual’s home, family home, or community settings typically supplement the primary care provided by the individual, family, or other unpaid caregivers. Whether you are looking for morning or afternoon staffing, weekend assistance, or even 24-hour support (with coordination by a Personal Assistance or Companion Care service), we offer positive and helpful staff with smiling faces!

Supportive In-Home services are available for adults and children living in their own home or family’s home who need assistance in activities of daily living such as eating, bathing, dressing and meal preparation. Each individual receives a specific number of hours per week according to their needs. Supports can also include leisure and community integration as well as the creation of safety nets to ensure the person can remain safely in their home for as long as possible.

Our Support In-Home program works within a variety of residential settings, from community-based to personal homes. From domestic activities to community outings, we do it all! We love meeting new people, and we look forward to developing a fun and complementary approach personalized to your unique medical and behavioral needs with dedicated 1:1 staffing tailored to match.

Silver Care LLC, Supportive In-Home services are defined here as services for individuals who are living at home with family members (i.e., immediate or extended family) or for individuals who are living on their own, with roommates or others without disabilities. Supportive In-Home may include supervision, teaching and/or assistance provided directly to or in support of the individual with developmental disabilities or is support of caregivers. Supportive In-Home are utilized to achieve and maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community for individuals with developmental disabilities. Supportive In-Home services are often provided in the individual’s family home, but community activities that originate from the home are also considered a part of the Supportive In-Home service.

Silver Care LLC offers a balanced program of care that meets the participants’ physical, emotional, health, and safety needs. Our employees are called Direct Support Professionals (DSP) who have the primary role of supporting participants on a day-to-day basis with routine personal needs, social support, and physical assistance in a wide range of activities to lead a self-directed life in their community.

Silver Care LLC will meet with the family and discuss the needs of the participant. An Individual Support Plan (ISP) is developed to meet the participant’s needs. Silver Care LLC, provides skills building training to support personal preferences as described in the Individualized Service Plan (ISP). The ISP uses person-centered planning (individual driven planning) to develop activities, supports, training, and supervision to assist individuals to maintain or improve their place in the community. DSP’s provides supervision, protection, and guidance in the home while working with the participant and implementing the ISP’s objectives.

Basic nursing care – First Aid, CPR, Medication Support (Reminders)

Training, Support & Assistance in eating, bathing, dressing, grooming, and other self-care skills
